Release Form ~ We are ONLY accepting online release forms with e-signature.
Stone Gate Farm Release Form: If you are going to participate in any activity at Stone Gate Farm as a rider, coach, parent, significant other, official, clinicians, spectator, volunteer or vendor, we need to have a current release form on file. From 2023 onward, once you have signed a release form you will not have to do it again unless there has been a change made to the release form. If that happens, you will be notified. zWe're happy yo announce that we have a new and easier to use Online Release form. You can check here to see if we have received the signed release form. They are sorted alphabetically. If you don't see yours there, scroll to the bottom of the page as that's where the most recent ones will show up.
His Way Farm release form
USEF/USEF Waiver and Release of Liability for MUST be filled out for anyone who is going to participate in any Recognized Horse Trials or Mini Trials as a rider, coach, parent, significant other, official, clinicians, spectator, or volunteer. These waivers need to be filled out for each competition and if not included with your entry they will need to be e-mailed to the secretary, Penne Colao.
Attention Trainers ~ If you are coaching or riding a client's horse you are required to submit a Certificate of Insurance with Stone Gate LLC aka Stone Gate Farm as an additional insured. Please use this link to upload the form.
Coggins ~ All horses coming to Stone Gate Farm have to submit a current coggins electronically. The coggins only needs to be submitted once/year unless it expires and then the new one will need to be uploaded.
